Cancellation Policy
Appointments MUST be cancelled within four (4) business hours of the scheduled appointment time or the following will be assessed:
Routine: $20
Physical: $40
Completion of Medical Forms
A $10 fee is required for EACH medical form, completed by a physician outside of an office visit, due at the time of a request
